Minimum Requirements:

• Must have State License to Apply
• 6 months Pharmacist experience preferred
• Bachelor of Science in Pharmacy required
• Registered Pharmacist License in state(s) of practice
• Minimum of two (2) years of recent experience in an acute-care setting or one (1) year of recent experience in home infusion therapy
• Experience with compounding infused drugs
• Proven record of evaluating and monitoring drug therapies for home care patients
• Flexible schedule with the ability to work on-call evenings and weekends on an as-scheduled basis

Assignment Details

• Rotating schedules: M-F 8:30-5:30, 8-5, and 9-6
• Scheduled on-call shifts from Friday 5:30 PM through Monday 8:30 AM, typically once a month
